That is the read. I am asking about the window between the read and the write — what makes that not a race?

Honestly — at our volume we never saw it. There is a unique constraint on (order_id, attempt), so the second insert fails and the worker retries into a no-op.

But I would not call the worker idempotent. The constraint is doing that work, not my code.
